GWEP Community Partners
The SFGWEP is one of the primary educational outreach centers for the College of Osteopathic Medicine at Nova Southeastern University. The Center has developed a model, community-based program that connects university faculty and students with Florida's populations 55 years and older to promote healthy and positive aging. To assure that the Center connects with the external community, it has established 11 community consortium partners to carry out its objectives:
ADRC - Aging & Disability Resource Center of Broward County
The Aging & Disability Resource Center (ADRC) is a nonprofit organization serving Broward County's senior residents, age 60 or older. The Aging and Disability Resource Center of Broward County plans, develops, coordinates, and evaluates programs; funds services; and is the prime advocate for residents of Broward County, Florida, 60 years of age or older. ADRC is one-stop source of information regarding services in Broward County, Florida, for seniors, persons with severe and persistent mental illness, their families, and caregivers.
AHEC - Area Health Education Centers NSU KPCOM
The mission of NSU’s (AHEC) Program is to improve the access and the quality of primary health care service to medically underserved communities by linking the resources of academic health centers with community-based health care providers. Nova Southeastern University’s Dr. Kiran C. Patel College of Osteopathic Medicine, the first medical school in the state of Florida to develop an AHEC Program, officially began its program in 1985. Since its inception, the program has worked to develop effective and comprehensive training programs that improve access to quality primary health care for Florida’s medically underserved rural and inner-city urban communities. By including training programs in community settings, we expose students to the challenges, rewards, and practice opportunities related to working in medically.
Aventura Hospital and Medical Center provides highly specialized tertiary care services, including cardiac surgery, cancer treatment and management, orthopedic and spine surgery, and neurosurgery. The hospital serves as a designated Level II Trauma Center and Comprehensive Stroke Center. Aventura Hospital is a statutory teaching hospital and houses multiple residency fellowship programs in underserved areas. Students have opportunities to work together while learning to provide valuable primary care services to the community.
CHI - Community Health of South Florida
A nonprofit health care organization providing affordable quality primary and behavioral health care services to the residents of South Florida. CHI operates 11 state-of-the-art primary care centers which offer quality comprehensive primary and behavioral health care services. Additionally, CHI is accredited by the Joint Commission and is accredited as a Level 3 Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). CHI offers a “one-stop-shop” model where the organization and delivery of quality services are seamless, affordable, accessible and culturally sensitive.

Florida Association of Community Health Centers
Since 1981, the Florida Association of Community Health Centers, Inc. (FACHC) has been the leading state to advocate for community-based health care programs. Focusing on Florida's Federally Qualified Community Health Centers, the Association plays a vital role in educating federal, state and local policymakers about issues relating to health care and the role of the health centers. The primary mission of FACHC is to improve access to quality health services by bringing together agencies, legislators, and key persons able to affect health care services.

LLI - Lifelong Learning Institute
Founded in 1977, the LLI serves lifelong learning passions of retired adults. the LLI was founded to complete the NSU dream of education spanning a whole lifetime, from preschool to older-adult learning. Through a unique, inviting environment, the LLI offers challenging educational lectures and social opportunities to connect with other like-minded individuals. through field trips and special events. LLI members audit classes at the university. Such experiences offer true intergenerational learning while sitting side by side with NSU students and faculty, sharing and learning from each other.
SE Florida Alzheimer’s Association
The Alzheimer's Association is the leading voluntary health organization in Alzheimer's care, support and research. Our mission is to eliminate Alzheimer's disease through the advancement of research; to provide and enhance care and support for all affected; and to reduce the risk of dementia through the promotion of brain health. The association works on a national and local level to provide care and support for all those affected by Alzheimer's and other dementias. As the largest nonprofit funder of Alzheimer's research, the Association is committed to advancing vital research toward methods of treatment, prevention and, ultimately, a cure. The Association is the leading voice for Alzheimer's disease advocacy, fighting for critical Alzheimer’s research and care initiatives at the state and federal level.
The South Florida Institute on Aging (SoFIA) mission is to improve economic and social insights and services to empower people as they age. Their vision is a world where everyone thrives as they age by being a think and act tank that empowers people as they age.
Urban Health Partnerships (UHP)
Urban Health Partnerships (UHP) is a non-profit organization whose mission is to invest in our communities by co-designing sustainable change and promoting equity and wellbeing across the lifespan. UHP focuses on promoting health and equity through policy, systems, and built and social environment change through community-driven approaches. UHP brings a unique lens to place-based and community initiatives as its multidisciplinary team is able to integrate multiple perspectives in public and mental health, engineering and transportation, community development, and equity to implement change.
