Forms that cannot be submitted online may in some cases be signed electronically using DocuSign or Adobe Reader, Mac Preview, or the Microsoft Edge Browser. If signing with Adobe, Mac Preview or Microsoft Edge, students (and others) represent that they intend and agree that such electronic signature(s) shall have the same binding effect as a handwritten signature, for purposes of validity, enforceability, and admissibility of this document. Any method of electronic signature other than as above-described may not be acceptable to NSU. Instructions for placing electronic signatures are provided in the How to Insert E-Signatures document. 

Forms may be emailed, mailed, faxed, or delivered in person to the One-Stop Shop located in the Horvitz Administration Building or your local campus.
