The Physician Assistant Program • Jacksonville Health Professions Division | 13 Personal Interviews Personal interviews are offered to the most-qualified applicants to assess interpersonal and communication skills, maturity, altruistic attitude, and commitment to the PA profession. Interviews are conducted at the NSU Jacksonville Campus during the months of September through February. Personal interviews are by invitation only. An invitation to interview is not a guarantee of admission. Application Withdrawal If, at any time, you wish to withdraw your application from consideration, please do so in writing. Box 299000 Fort Lauderdale, Florida 33329-9905 • Send two letters of recommendation. ■ one letter from a physician assistant ■ one letter from a health care professional such as a physician, physician assistant, nurse practitioner, registered nurse, physical therapist, or occupational therapist Recommendations submitted by relatives, personal health care providers, family, or personal friends of the family are not acceptable. • Send GRE score. Official Graduate Record Exam (GRE) scores must be submitted directly to CASPA. Your GRE test scores must be less than five years old and must be taken early enough for official scores to be received by the supplemental application deadline. The school code number for NSU’s PA—Jacksonville Program is 0952. 3. Once the CASPA application has been received by Nova Southeastern University, a supplemental application will be available online. You must submit a nonrefundable, $50 application fee with your application. In order to be considered for the May entering class, supple- mental applications must be received by January 15. Applica- tions are reviewed upon receipt of all required documents by the deadline. At minimum, the following must be received: • CASPA application • official GRE scores • NSU supplemental application and $50, nonrefundable application fee Transcripts can be sent electronically from the originating college/ university to the following address: electronictranscripts@nova.edu .
