Nova Southeastern University | 21 INTERNATIONAL DENTAL GRADUATE PROGRAM APPLICATION PROCEDURES 1. Mail materials to the Enrollment Processing Services (EPS) by January 1. Applicants must send • a completed College of Dental Medicine application for the International Dental Graduate Program • a $50, nonrefundable application fee • an official score from the Test of English as a Foreign Language (TOEFL) 2. Submit official transcript(s). Arrange for an official transcript to be sent directly from each college, profes- sional school, or university attended to the EPS by January 1, if coursework was taken at a U.S. institution. Transcripts can be sent electronically from the originating college/university to electronictranscripts@nova.edu . 3. Submit official course-by-course evalu- ation and cumulative GPA. Coursework taken at foreign institutions must be evaluated for U.S. institution equivalence by an approved National Association of Credential Evaluation Services (NACES) organization, such as one of the services listed below. An official course-by-course evaluation with a cumulative grade point average must be sent directly from the evalua- tion service to NSU’s EPS. 4. Submit scores for Part I of the National Board exam. Request the scores be forwarded from the secretary of the National Board of Dental Examiners, located at 211 East Chicago Avenue, Chicago, IL 60611, to the EPS. Please submit Part II scores also, if taken. 5. Submit three letters of evaluation to NSU’s Enrollment Processing Services. Letters may be completed by dental school faculty members who are well acquainted with the applicant’s abilities or by individuals who can provide infor- mation relevant to his or her potential.
